You are welcome to review these forms. Additionally, you can download and print these intake forms before our initial session. Before we begin therapy, please complete the requested forms, sign and date them. I will have these forms as well, for you in hard copy to review and sign, on a clipboard, in the waiting room, before your first appointment. If you have any questions regarding any information provided in these forms, you do not have to sign them and we will discuss any concerns you have at our first session.
The client information form and the statement of confidentiality will both need to be completed and signed. The HIPAA form needs to be reviewed. When you sign the Statement of Understanding form, you will be acknowledging that you have also reviewed and understand the contents of the HIPAA agreement. At any point in the process, you are welcome to ask questions.
